Community Health Needs Assessment and Strategic Implementation Plan

On a regular basis, the University of Chicago Medicine conducts a Community Health Needs Assessment and develops a strategic implementation plan.

Community Health Needs Assessment

The University of Chicago Medicine has completed a Community Health Needs Assessment in order to improve the health of residents in our service area.  The Community Health Needs Assessments are a systematic, data-driven approach to determining the health status, behaviors and needs of these residents. Required by the Affordable Care Act, a Community Health Needs Assessment enables communities to identify issues of greatest concern and decide how to allocate resources to address those issues, thereby making the greatest possible impact on community health status. The medical center, in collaboration with the Metropolitan Chicago Healthcare Council, sponsored the report. Informed by the Community Health Needs Assessment, the following health priority areas were identified: breast and colorectal cancer and diabetes for adults; obesity and asthma for children and adolescents; and, violence prevention and sexually transmitted infections/HIV.

The medical center has set the following strategic goals and objectives to address the health priority areas identified through the Community Health Needs Assessment. These health needs are:

  • Breast/colorectal cancer
  • Diabetes
  • Pediatric asthma
  • Pediatric obesity
  • Violence prevention
  • Sexually transmitted infections/HIV

UChicago Medicine is committed to the following goals and objectives to improve health across these areas.
