The South Side Healthcare Collaborative (SSHC), a core component of the Urban Health Initiative, has helped many people find medical homes. See below for videos featuring patients who have found the health care they needed through the SSHC:

Sheila's Story


Before finding a medical home at the Chicago Family Health Center, Sheila Aguilera visited emergency rooms for health care. Now, this busy mom, full-time worker and student receives coordinated care for her diabetes, including personalized nutritional advice. Semeca Johnson, a patient advocate, is also featured in this video and speaks about the benefits of finding a medical home. (5 minutes, 23 seconds)

Ben's Story


For Ben Holliman, a routine PSA test performed at Komed Holman Health Center ultimately led to a diagnosis of prostate cancer. Fortunately, Ben's cancer was found in time for it to be successfully treated with surgery. Preventive care offered at Komed and other community health centers is enabling Ben and people like him to look forward to many healthy years ahead. "Komed saved my life," he says. (5 minutes)

Barbara's Story


Barbara Hunter found a medical home for herself, her daughter and grandson at the Friend Family Health Center (FFHC). FFHC is close to Barbara's home, providing a convenient place to receive coordinated care for the whole family. Barbara's physician, Leah Durst, MD, is also featured in this video. Irene's Story


Because Irene Hurley had no health insurance, she was concerned she would not find the care she needed to treat her asthma and bronchitis. Now, Irene has a medical home at the Chicago Family Health Center. Fees for health care provided at the center are determined on a sliding scale, taking into account each person's ability to pay.
