Q. How can I get an appointment?

Q. Will you need my medical records from other health care providers?
A. If you would like our physicians to review your records, we can assist you in obtaining them.

Q. Are there any dietary restrictions prior to my appointment?
A. Please do not eat anything, and drink only water, after midnight the night before your visit. We will provide you with a light breakfast after your blood has been drawn for lab tests.

Q. What should I wear?
A. We recommend that you wear casual clothing and tennis or gym shoes for the day. This will make it easier to perform the exercise stress test.

Q. Where is the Program for Executive Health located?
A. We are located in Suite 4A in the Center for Advanced Medicine at 5758 South Maryland Avenue in Chicago. Our phone number is (773) 834-9000.

Q. Where can I park at the University of Chicago Hospitals?
A. It is easiest to use our valet parking service. Simply drive to the front entrance of the Center for Advanced Medicine and give your car to a valet. If you prefer to park your own car, there is a parking garage on Maryland Avenue.

Q. How long is a cab ride from the Loop?
A. The average travel time is 15 minutes. But it may take longer during high traffic hours.

Q. How long will my visit to the University of Chicago take?
A. Your day here starts at 7:45 a.m. and is completed by approximately 4 p.m.

Q. Is my Executive Health physician a specialist?
A. Our Executive Health physicians are all a board-certified in internal medicine. They are trained to diagnose and treat a wide range of health problems.

Q. Is there a secure place in your office for me to put my belongings?
A. The Executive Health Suite is secure at all times, and you can feel comfortable leaving your personal items here.

Q. Will I have access to a phone?
A. We provide you with phone and fax access for your convenience.
Q. Should I bring work to do?
A. Our goal is to minimize any wait times so you won't have much free time.

Q. Will I have a chance to eat breakfast? What about lunch?
A. You will be served a light continental breakfast after your blood is drawn for lab tests. Lunch will be provided in the Executive Health Suite.

The End of Your Visit ... and After
Q. When will I get the results of my lab tests?
A. You will generally receive all test results at the end of your visit here. Results from any tests still outstanding will be sent to you.

Q. If I need follow-up testing, how do you arrange it?
A. Our staff will assist you in making initial follow-up appointments that may be recommended by your Executive Health physician.

Q. Will the results of my exam be sent to my regular physician?
A. Only at your request. If you would like us to, we would be happy to forward test results to your primary care physician.

Q. How does the Program for Executive Health ensure that my health information is kept confidential?
A. We take patient privacy very seriously at the University of Chicago Hospitals. We have strict policies and procedures that dictate the handling of paper and electronic information that contains confidential data.

Q. What methods of payment are acceptable?
A. We accept personal checks or credit cards (American Express, Visa, Mastercard).

Confidentiality
As you would expect, all findings in the Program for Executive Health are held in strictest confidence. Test results and your medical report are sent to others, such as your personal primary care physician, only at your request.
