Request an Appointment Online

Before scheduling an appointment, all patients must first register online through myHealthManager--a secure private Web portal set up specifically for bariatric surgery appointment requests. This is the only way to request your first appointment.

To register, you will need to enter information about your medical history. You will also need to know if your insurance plan covers weight loss surgery. Please confirm this information with your insurance provider before starting the registration process.

Once you have completed the online registration process, our staff will review your information. We will contact you within two weeks to let you know if you are an appropriate candidate for bariatric surgery. At that time, we will schedule you for an information session about our program and our weight loss surgery options. We will also schedule you for evaluations with a surgeon, a dietitian, and a mental health professional.

There is a $125 registration fee, but you will only be charged if you have surgery.
