Our clinic is located in the Community Diagnostic Center, which is 1.5 miles south of Community Hospital, just off Calumet Avenue. On-site parking is available. Upon entering the building, check-in at the first floor reception area.

From Indiana: Take I-94 west to Calumet Avenue via exit 1 toward Munster (3.1 miles). Turn right onto Palmer C. Singleton Jr. Drive (0.1 miles). Turn left onto Donald S. Powers Drive (0 miles). 10020 Donald S Powers Drive is on the right.
